Yes. It fits the 72, c10 frame. Make sure u trim off that spring reinforcement plate so the c notch fits flat on the frame. It’ll be evident when you get it and put it in place.

Does it come with a template and measurement instructions

It comes with the 2 c-notches and hardware bolts except for the template and measurements instructions but can search on YouTube on how to install the c-notch that’s how I install mine on my 65 C10 truck.
