what kind of batteries does it use?

Two AAA batteries. Readily available anywhere. But in several years of running I've never had to replace them - so battery life is very, very long!

Does it fold up? What is the dimension after being folded up.

No, this clock does not fold. It is a convenient shape for so-called "suitcase tetris", however. I have always taken it with me and it has always fit in my bag.

No - it sits like pictured but does not fold. It can be tucked among clothing easily though. So far so good - mine is holding up well and I'd recommend it.

does the alarm shut off automatically or do I need to turn it off?

It goes for one minute then shuts itself off.
