What country is this this bottle made in? I find it odd that does not say on the box or the bottle where it is made.

Sellers response actually has a lot of good info. Cap and bottle are made in China. Choice of materials is safer than many other options.

Is the opening wide enough for ice cubes?

Not any more! I've bought several of these bottles over the years, for my family and as gifts, and always LOVED being able to fit ice cubes in it and to be able to clean it with my long brush. But the mouth now is slightly smaller. Hard to get ice cubes in it (you can, but have to kind of pound them in!) and I can't get my brush in to clean it. I don't know what's going on with the bottles, seems like a manufacturing issue going on, but I hope the old model comes back soon!
