Q: My husband is size 40", which is best size to buy large or xlarge?

To be on the safe side I'd probully get the xl it does have a elastic belt to it .but I found it to be quite tight still .I'm in women's size 20 bottoms hope this helps

Q: Is it meant for wearing under your underwear?

I think this would be a question of choice, but for me, I wear brief snug underwear and the support goes on afterwards, therefore it is not necessary to have to continually wash the support . Obviously certain functions are more problematic this way round, but one can get use to it! ( not sure a picture or video would be appropriate?)

Q: Is this good to use while weight training in the gym

Have used to good effect while doing gardening/diy jobs that involve lifting and bending but not sure about serious gym/weight training with a hernia without proper medical advice!
