How do I connect this to a 3.5mm input from a Laptop or an iPhone?

That Lavalier mic is designed to be used with either a wireless Lavalier microphone pack or it need a small shure power supply. It will not hook up directly to a computer. You might want to see if you can find a USB Lavalier microphone. If you type that into the Amazon search engine it will show you a few choices.

Does it include the pop filter and the tie clip?

How long is the cord?

The cord is a standard lavalier microphone chord, about 3 feet.

I am looking for this same microphone in a headset version (over the ear) rather than a lavalier. Any idea what that is called? It's not a wl184.

There is not (to my knowledge) a Shure headset that uses the same capsule as their lavalier microphones. The closest thing I could find was the MX153 which is single ear omnidirectional headset that is compatible with the Shure wireless systems (using the TA4 Connector). Another option would be the Countryman e6, e6i or h6. I would recommend a cardioid or supercardioid pattern if you are planning on using it for live applications though, not omni. Personally I would prefer a DPA D:Fine headset with the D:Fine slim capsule with a cardioid pattern and 2 ear design, but it's significantly more expensive. There are also plenty of budget headsets you could check out, just make sure they have a TA4 connector and they should say compatible with shure systems in the description.
