Why is speed described as 0-1200 RPM and review says it starts at either 250 or 600 RPM? I need something with true variable speed.

It definitely does not have a gradual start, as some have indicated in the reviews. The lightest first contact in the trigger yields around 300 RPM's unloaded on my drill (I checked with my touch-tach). With the level of power this drill offers, even at this lowest trigger position the drill won't stall without a large amount of load. If you need delicate low-speed operation such as fastener driving, look for a smaller drill (perhaps a 3/8" VSR?). If you need a drill that will work continuously poking 3/8" to 13/16" holes in up to 3/8" thick steel as I did, this is your drill.

Has anyone found a good case for this drill?

I just use a generic tool bag for the drill.

Any masonary bit included ?

I purchased the drill only, so I do not have a hands on answer regarding the drill bits. However, the drill bit collection packaging does not indicate a masonry bit (and I would not expect one in a generic small bit collection).

Will this drill be able to bore out 1 1/4" holes in 4x4 studs?

Have not done exactly what you need, but I have used it to bore 1" holes in 2x4s no problem. It's quite powerful and the "handle" helps w/ stability.
