Has anyone tried to use this bag as a carry on?

Good for a carry on as long as you stay with a 50L bag and no larger. It’s the same size as a medium duffel bag. The bag is soft so it can be scrunched, squeezed or pushed into a space. If over pack, a good suggestion is to pack a small-medium foldable duffle bag in your bag so you can take out any excessive items and put them in there for the under seat compartment. Fits enough for up to seven days of clothing (if you roll your clothes or vacuum seal). Will fit laptop as well if you pack along the back strap side (easy pull in an out for TSA). I prefer this bag as my go to carry on as it has fit on even the smallest commercial planes. Just throw it on your back and go!

Does it have a pocket for a water bladder

Yes, It does have a "laptop sleeve" on the inside of the main compartment which can hold a bladder where the hose can be fed out of the top of the bag and hooked to an attachment on the front right shoulder strap.

Does it have an internal frame?

Dear, it haven't an internal frame

Not at all waterproof. It does come with a nice rain cover which can wrap all around the outside of the bag.
