How do you determine what oz size to buy....4.5 oz or 7.5 oz?

Notice the price and then the price per ounce. It is twice the price per ounce for the bigger ones. I am not talking twice the price for the whole bottle which would make sense, but twice the amount of money for each ounce. So, the person who said the 3 pack 4.5 oz is less money than the 7.5 oz is only right IF the amount PER ounce is less.

Are you supposed to squeeze the bottle side to side or from the bottom going down?

Start by squeezing the bottle side to side, then squeeze and roll up from the bottom, the way you would a tube of toothpaste. Have the cap handy to replace on the tip fast.

The seller just stated latex and gluten free. Not sure about the BPA, since I honestly don't read anything on the back when I'm desperate to go...
