Q: What does 3 pack mean? Is it just two call buttons and one door bell? 3 pack is misleading.

There are 2 buttons so you can have them in separate rooms and 1 bell

Q: can i purchase more push buttons and they will all work on the same receiver?

Unfortunately this system cannot be expanded. If you are looking for an expandable call system, we recommend the Call Button & Remote Alarm http://www.amazon.com/dp/B0075GPN9U, with as many Wireless Call Buttons http://www.amazon.com/dp/B00DNGVT3U as you would like. Note that the Remote Alarm will not tell you which button triggered the alarm. If you want that type of system, we recommend this Wireless Central Monitoring Call System http://www.amazon.com/dp/B009ZB9218. You can add the Wireless Call Buttons to it as well. Q: Is an AC adapter available for the units? If not what type of batteries are required?

Not that I am aware of. The call button uses a small battery A235 12V. It lasts a long time. The Pager uses AA batteries. We had the units over a year and never replaced the call button batteries. We replaced the Pager batteries twice.

Q: Is there a connection for a strap so it can be worn around your neck?

Yes, both call buttons come with a lanyard. You can hang them around the neck, or you can set them in a holster that sticks to the wall (I used Command strips instead of the provided sticky tabs so the holster is easily removable if I want to move it). Each button comes with a holster and a lanyard so you can choose. We have one without the lanyard stuck to the wall in the bathroom. The other is on a lanyard that my mother can wear around her neck but then we put it in the holster by her bed so it is accessible to her in the night.
