Stimulator "Pulsetto"
I got problems with sleeping approximately a year ago. I waked up in the middle of the night with no apparent reason and then couldn't fall asleep for a long time. In the evenings I worried, fearing a possible awakening. In the morning I woke up not having slept enough and exhausted. I was afraid to drive a car. Friends advised me to take sleeping pills and do calming procedures. Searching for the cause of my problem on the Internet, I found that it was a problem with the vagus nerve. The symptoms were almost identical and I started to look for a ways to treat this ailment. A month ago, I came across a description of the Pulsetto stimulator. I compared the characteristics of this device with similar ones and decided to try it and I was right. The result is simply amazing. I did the procedure once a day for 4 minutes and after four days I started sleeping whole night, no waking up. My daytime mood and family relationships have noticeably improved. The device is very convenient, compact, easy to charge, the manual is very detailed and clear.

Surprisingly effective
I was skeptical, but have had success with TENS for pain management in the past so I gave it a try. Post menopause, this has been my most challenging year yet for sleep quality. I typically wake up every couple of hours to hot flashes. I’ve been using Pulsetto for just over 30 days and I’ve noticed I can now go 5-7 hours without waking up and when I do wake up, I generally go back to sleep fairly quickly. I’m feeling so much better rested, and the hot flashes have been less frequent and less intense. I’ve found that two back-to-back ‘sleep’ sessions at a 7 intensity works best for me. I react very poorly to many prescription medicines so this has been a godsend for me. I haven’t slept this well in over 5 years!

Not worth the price tag
I've been bombarded with ads on social media for various as-seen-on-IG influencer-shilled health and wellness "products" recently, and to transcutaneous vagal nerve stimulation (tVNS)'s credit, it does seem to be one for which there appears to be some benefit vs sham stimulation for certain conditions. I decided to give Pulsetto a try since its negative reviews only mentioned non-functional devices or problems with fit.Another issue I noticed when trying to research tVNS online is that there isn't clear reporting on how the treatment is supposed to feel. I see that Pulsetto advertises immediate changes after one use (4 minutes) and/or changes over two weeks of twice a day usage, but this hasn't been the case for me. I have an anxiety disorder with associated symptoms like depression, IBS, poor sleep, chronic stress, etc., and I take prescription medication to manage these symptoms including an SSRI, prokinetic drug, a low dose of beta blocker, etc.The Pulsetto's settings by default are an intensity of "5" and stimulation on the right and left electrodes, which seems to be at odds with reasoning I've seen in scientific papers with regard to implanted vagal stimulators, e.g. "The right vagus nerve predominantly supplies the atria and SA node and provides cardioinhibitory tone. In contrast, stimulation of the left vagal nerve, which innervates the ventricles, has not been shown to cause any significant change in cardiac rhythm or rate. This observation is the rationale for implanting vagal nerve stimulator leads in proximity to the left vagus nerve because stimulation of the right vagal nerve might lead to bradycardia." (Muppidi 2011)Each Pulsetto session type is given a name in the app like "anxiety", "burnout", etc. I don't know what the differences between these modes are other than duration, and that Stress/Anxiety/Pain are accompanied by warnings to only do them twice a day, while Sleep does not carry a warning, and Burnout may be used without limit.The first time I used Pulsetto, the session turned on the electrical stimulation at intensity 5 a couple of seconds in, and I felt like I'd been hit in the throat and that my facial muscles were spasming and being dragged to one side of my face. I desperately turned down the intensity on the smartphone app, which stepped down to the lowest level after a short delay. A $30 TENS unit will start at zero and instruct the user to increase the intensity until they just perceive the tingling sensation. For subsequent sessions, I changed the default settings to intensity "1" and left side electrode only. Intensity 1 still comes on suddenly, but it merely produces a tingling sensation in the skin and some slight tightness/pulling feeling in my lips and jaw.I haven't experienced any changes in my mood or sleep on a day-to-day basis after using the Pulsetto as directed (with the gel, twice a day, etc.). It does produce a tingling sensation/stimulates the muscles while used, but this does not provide the promised benefits (changes to heart rate, reduced feelings of anxiety and burnout, improved sleep quality, etc.) for me. The app's various modes do not provide details on the differences between them. I can't recommend Pulsetto as it has far too high a price tag, even on sale at 268 USD. I remain curious in regard to tVNS and TENS in general, but not enough to justify this amount of money.
