Does this come with the lid that covers the bottles?

Yes, the bottles each have a clear, plastic top that covers the nipple and prevents leaking.

After washing, how do I know which nipple is which? Are there any markings?

No markings, but slow nipples have 1 hole, medium has 2, and fast has 3. Easiest way to tell outside of inspecting the nipple is to turn it upside down and watch how many droplets emerge at once.

Should I get a sterilizer with these? Which one?

Yes, you should. Any sterilizer is fine. I have a microwave one and works fine
