I’m 5`1 145lbs and I got the regular one, 104. Price, it fits me fine

Q: How can I get a large in this? Patient 6 feet tall and 154 lbs

This should fit, but it does not give good back support. if more back support is needed, look for one that will cover from top of sholder to tip of tail bone, there are several out there by different manufactures. we got ours from "Drive"
