Q: Would these be big and soft enough to use as a head covering for cancer patient

No these are not big enough for a head covering. They are a little bigger than a full size Kleenex tissue.

Q: Do the colors bleed when you wash them?

I have not washed them yet I put them in with sympathy cards (for loss of pet or person) so I don't know but I imagine if you wash them in cold water they would be fine. I also heard if you soak material in white vinegar it will "set the color" and they won't bleed. Sorry that is all I know.

Q: Can you sew lace trimming on them

I believe you could sew lace around the ends!

Q: Im wanting to use them for napkins. Would they be appropriate for that use?

Yes. I bought them to use as napkins and love that they are small and thin. This makes them easy to wash by hand quickly. I usually use the sams one for 1-2 days, then throw it in the laundry. But I'll hand wash in the sink if it's something like bbq or pasta sauce just to prevent stains.
