I have not tried it with cold water yet. But for my purposes it is quite comfortable and I feel like it's going to last quite some time. I do not suggest using boiling water, the manufacturer says do not use boiling water so just as hot as I can get it from the tap suits me fine. I really do appreciate the horseshoe design because I've tried using neck wraps before and they just don't seem to ever get close enough to the area where I need the heat. Extra cost of $6 - 9 seems to be for sleeve cover which u need IF u can get it on..I will slip stitch the ripped seam..
