Does this supplement use cordyceps extract powder, or are the pills filled with ground up cordyceps?

The produce used cordyceps extract powder which identify has been verified via DNA. It is standardized as the total polysaccharide Content is 65% and the level of 1,3- 1,6 Beta Glucan is 30%. Hope it helps.

Says manufatured in fda registered facility yet says on this page all statements have not been reviewed by fda?

An FDA registered facility and an FDA reviewed statement are two completely different things. An FDA registered facility means that it isn't likely that the product is being made in someone's backyard but that the processing is clean and modern. An FDA reviewed statement means a regulated statement submitted for testing to the FDA to treat, prevent, cure a health issue (generally). This brand is my go-to. With Cordyceps you will know if they work within a day or two.

What extraction method is used?

It doesn't specify the extraction method.
