Q: Two questions please: 1. How is this tile applied to the pool deck? 2. How do you prevent this from becoming a trip hazard?

They are actually put into the deck. Place the marker where you want it to be. Mark the outline of the tile. With a small grinder and a concrete wheel, cut/ grind a cavity just over 1/4" deep. The tile can then be placed in with thinset and it will be level with the deck eliminating any trip hazard.

Q: how is it applied to the pool side wall as depicted in the photo?

Normally installed with the waterline tile when the pool is being remodeled or built. They can be installed as a retrofit also. Using a diamond blade on a 4 1/2 " angle grinder, cut the grout on all 4 sides of the tile you are replacing. Then cut an "X" in the face of the tile. This will allow the tile to be removed. Clean out the old thinset and install the new depth marker using standard thinset. Regrout around the tile.
