Q: does the heating element go on top of the white insert or on the bottom of it closest to the floor?

I have several of the Marunda heated pads but not the bed. Best guess, looking at the image, is that the cord exits closest to the floor. FWIW, I really like their product. My cats adore them.

Q: Can you use an extension cord?

This unit uses less power than a 60 watt lamp. You can absolutely use an extension cord. The only liability issue is probably related to an animal chewing on the extension cord.

Q: Can you buy extra covers for this heating pad?

Not that I have seen. But they are very easily removed for laundering.

Q: Can this heating pad be folded while it's on?

I don't think it's safe to fold it when turned on.
