Is the card flashed to IT mode before shipping? It seems to say that in the title, but one review says it was not.

Mine was flashed into IT mode when I got it, just plugged it in, attached the cables and was up and running.

Will this work with just 4 PCI-e lanes available (albeit in a x16 slot)?

It should work. But speed will be downgraded to PCIe x4.
