no, it does not have a zipper, button or velcro. There's a small opening at the top of your underwear in the inside and the pocket goes all the way down below your groin. For the most part, gravity keeps everything in. If you plan to do something that turns you upside down, the fact that it's on the inside and below the waistband, i'm sure it will still stay in there.
