Does this kit come with three or two brass plated bolts for the tank to bowl connection? This identical kit in HD has only two. This picture shows 3.

Only 2 brass bolts but the kit comes with extra rubber washers for toilets that have 3 bolts. The instruction said you would have to reuse one of your old ones for the 3rd brass bolt. I have mansfield toilet, which is 3 bolts. I didn't use the brass bolts provided in this kit. Instead, I bought "Mansfield 630-0204 2" SmartFasten Tank to Bowl Kit", which is so much easier to install and works flawlessly!

Does this work with a Jacuzzi BK37 toilet?

If it's a 3" flush valve, use Fluidmaster 400A. If a 2" flush valve, this product will work.JohnVermont Outlet True Value

Anyone Know of this will replace an OLD Moen toilet assembly?

Yes, I believe it will! This is an excellent product. I have had no trouble since installing mine except you must have clean flush water available. Resin fines from my water softener caused this products valve to seep until it was flushed with clean water.

Will this kit work for a lever on the side ? All instructions seems to be for a lever on the front.
