Does this machine includes a duo-cutter to make three varieties of pasta; and include the attachment for lasagne, fettuccini and tagliatelle?

You can make lasagne with the basic machine and yes there is a duo cutter included to make tagliatelle and fetttuccini.

The finer blades seem to make pasta much thinner than we would expect spaghetti to be. Is it actually an "angel hair" cutter?

No, it's spaghetti. There is a separate angel hair attachment available to purchase separately I think. The one that comes with it is Spaghetti. For what it's worth, I recommend you buy this pasta machine. I've used mine several times already and it's much better than the two el-cheapo ones I previously ran into the ground. Hope this helps.

Is there any plastic on the rollers or is this all metal

The roller itself is metal but where the crank handle fits into is plastic and unfortunately with ours the plastic inside became brittle after time and snapped as we turned the handle. Such a shame as there's no way to fix it so the machine was binned. If it had been all metal then it wouldn't have broken!
