Does it come with the plastic screw ring to secure it to the under side of the cistern?

This is just the button assembly. I think it came with a plastic screw to attach it to the flushing cartridge below, to hold the cistern lid firm.

Are these just cheap plastic with chrome plating which wears off?

Yes ... Ours has broke again ... Need to buy another
