What exactly does the app do for the watch besides set the time?

The app primarily syncs the time to your phone but also offers a secondary time zone for travelers as well as alarm and timer but obviously your phone already has those features. The only other thing the app has is the guide that is the instructions for use document. So essentially you are correct, the app mostly is making sure time is exact by syncing with your phone but even so you have to push the red button to update it for daylight saving time changes.

What does “Battery life: 5 months” means?

This is a solar power watch, if left in a drawer for example it should have power reserve lasting that long without exposure to light
