Q: the xx large what waist size does that cover??

Style 76925A it covers waist 44"-46"

Q: Does anyone know which photo is more accurate? (With or without the grey stripe above the Hanes logo)

There is no grey stripe. My items arrived just like the photo of the different shades of blue.

Q: How many of them in the pack are actually 100% cotton and not the heather? I prefer the all cotton solid colors.

Mine were all 100% cotton. There were no heathers in my pack. I got dark blue, light blue, green, solid gray, and tomato red, all cotton. I also ordered a different set of underwear, and 2 out of the 5 were heather gray, 75% cotton 25% polyester.
