Q: Would this work on a floor lamp?

It might. It would depend on the floor lamp. I am not sure you would get sufficient coverage of the bulb in that scenario. The curve of the bowl is very shallow so I am thinking that the bulk of the bulb in the floor lamp would be exposed and thus resulting in an uncomfortable and unattractive situation. Of course, for some floor lamps, it would work. You could also get one of those extensions used for light bulbs which allows them to be used as outlets. That might raise the height of the bulb from the lamp sufficiently that you could use a small clip on shade. The answer to your question could go either way on this one depending on the actual design of your floor lamp. I have used this sort of clip on for a small table lamp which did not have a shade - the kind that are about 15 inches high without counting the shade. In that case it works well because the table lamp is small enough that you are looking down on it when seated at your desk so the light bulb glare is not an issue

Q: Would it work over a fixture where 1 bulb just hangs? Confused how this works

A wire fixture clamps onto a vertically hanging standard light bulb. the fixture has a bolt at the end onto which the plastic cover is a affixed with a finial nut. It's self evident when yo open the package.

Q: I have purchased this shade twice now, and both times it came without the clip! is this normal?

I ordered several clip ons and they all came with the clips. There are also styles that attach with a cap to the stem coming out of the fixture itself. Make sure you didn't accidentally order that style. They wouldn't sell a clip on without the clip. Good luck.
