Has there been any issues with the frame or wheels failing? What type of warranty is offered?

I DO not really know about the warranty, but I am a big man ( 435 pounds) and do not have any trouble rolling around on this cart. Plus also the seat is a comfortable pad, does not make your butt go numb. I have had these seats for about 3 months now an they have worked out great for me. I hope this helps your question.

In what country is this manufactured?

Not sure but the company headquarters is out of Nashville.

Can the backrest be removed, or if the stool requires assembly, can the stool be assembled without the backrest?

The backrest is completely optional in your assembly.
