Does the metal part go all the way through the handle?

Yes, the main part of our car door assist handle is solid aluminum alloy handle, and the red rubber just covers the metal part.

Can i use this for Nissan Altima 2019? The tip of the last one that i ordered from amazon was wider than the door latch so it did not fit.

I hated this item. It was so loose in the door catch that it was dangerous. I bought it for a Subaru.

Can this be used perpendicular to the car door?

Seems it is being used perpendicular to the car door...when door is opened. It can be turned in the latch, i.e. 90 degrees from the ad picture. Not quite sure what the issue is... simple tool to assist entry/exit to the seat in the car. We've found the auto cane to be quite helpful...
