What kind of plastic bottle does it come in? The recycle symbol on the bottom is so poorly embossed that it is unreadable.

Is this nsf approved ?

I cannot find anything on the label that indicates NSF approval, and while I don't know for certain that it is not NSF approved, I suspect that it is not. According to the label, it is produced in the USA at a "GMP & FDA" inspected facility, but a legal disclaimer on the product's Amazon listing page states that it has not been tested by the FDA.

Is this truly reacted or not? the first bottle i purchased says reacted magnesium on the bottle. the new bottle does not.

Yes, this is still the same exact product, but with a new label. If you look at the supplement facts panel you will see that this product still contains di-magnesium malate, and magnesium bisglycinate chelate. Both of these are types of reacted magnesium.
