I have a 44 in chest and a 32 in waist. What do you recommend?

I recommend a medium, the size runs a little large.

Normally the small starts at 38 not 40, like this https://www.amazon.com/darcchic-double-breasted-waistcoat-aristocrat-steampunk/dp/b079l5m3h1/ ?

I recommend a size small, the size runs big.

I am 6’1 and weigh 210 what size?

My husband is 5'11 and 185lbs, he got the extra large size and it fit nicely, not too snug on the tummy.

I'm looking to buy, I'm 5'4" and 3/4 at 250 lbs and my gut sticks out more than I am wide. Any suggestions on size?

I suppose you could. I wear it as formal attire. It is a Victorian suit vest in Gothic style.
