Is it necessary to tighten the screw to the wall to be "stable?" In other words, can i leave the screw out about 1/8" for hanging pictures via wire?

No a problem. Even 3/16" or 1/4"

Will this work on a hollow wooden ( bifold )closet door to replace a hinge ?
