Q: Does it flow smoothly? Do you have to shake it to continue working?

Yes, it flow very nice, no need to shake, is plastic outside (it seem like crystal inside but I cannot tell), very sturdy and well sized (about 6.5" tall).

Q: can this be used in shower? looking for timer without suction cup

No suction cup but I'm sure it could be in the shower since it's made out of plastic.

Q: Can the timer be removed from the outter case?

We didn't try to remove the timer from the outer case, as this was exactly what I was looking for. The outer case gives extra protection. Check with the manufacturer about how it was put together. My guess is if you remove the outer case the bases (top and bottom) will no longer enclose the sand.
