Does this carry on have an internal system for garments?

It has a shirt/suit folder on the inside - this is removable. In the main case, there are compression straps and it also comes with a compression cube type system for t-shirts etc.

Can you strap on a backpack on the top?

Not, it doesn't tip over much, even when expanded (you're right, the expansion is at the top). I usually pack heavier things at the bottom, so tipping hasn't been a problem for me with this bag.
