Does this unit come with a battery installed?

No Come with but not installed I’m thinking about returning it. It is too loud with certain noises

What size battery is required?

it is a P13 battery. that being said each battery maker puts its own letters with the 13. just look in hearing aids with the 13 number, also the tabs on the hearing aids will be orange no matter who makes the P13. example duracell DA13H Energizer AC13EZ. If you go to microbattery.com it will give you a cross reference chart with the numbers.
