I am confused about the configuration. Do I just connect 1 unit to the router and the other 2 connect via wi-fi or do I have to ethernet to all 3 unit

You can do either. However if you do use ethernet for all 3 there might be a problem if you connect more than one directly to the router or via a switch. It can create a network loop. So if you do want to use ethernet for more than one unit, connect the second and third ethernet cables between units. Each unit has 3 ethernet sockets, so you can do this. I think that if you have a switch that does detect loops you don't have to worry, but my switches didn't work.

I have an out building approximately 30m from the back of my hse and 60m from the router. Can this be used to extend my Wi-Fi to the out building?

I have installed this mesh system in my home. It works very well in the house, and at the bottom of the garden. The distance from the nearest mesh boxes is about 35metres. I have testing the signal strength and it is good. No guarantees that it will work across the distances in the questioner's situation but I have been very happy with this system.

Can you setup multiple wifi networks ssids eg I have one for devices and one for people (so I can change the people password but not have to reset all

Yes, we use main network for peoples gadgets, TVs, laptops, Firestick etc Then Guest network at 2.4 GHz only for smart devices like cameras, doorbell etc. Easy to do too
