Q: Is the 128 oz mark the top of the bottle or the 7am mark?

For the Gallon 128oz bottle you have to fill it to the tippy top rim to reach 128oz. Literally to the top just before it starts to spill out LOL I filled this gallon bottle using 10oz water bottles. It took 13/10oz bottles to fill it completely to the tippy top rim. It was about 2oz of water left in the 13th bottle. It took 10/10oz bottles to reach the 7am marker which equals 100oz. It took just shy of 3 more 10oz bottles to fill it to the tippy top rim which was 28oz. Filling the bottle to the 7am line is 100oz From the 7am line to the tippy top is 28oz Between each 2hr line marker (bottom to 9pm-7pm-5pm-3pm-1pm-11am-9am-7am) is 12.5oz. That's 8 line markers x 12.5oz = 100oz at the 7am marker. I hope I explained this fully and it was helpful.

Q: Does it come with a straw?

It has a little fold up "straw" that you can drink through, but it's only about an inch tall. It's not like a regular straw. I wouldn't recommend this product though. It's very thin cheap plastic and breaks easily. Mine fell off my car seat on to the carpeted floorboard and shattered. :(

Q: If I purchase the gallon size with the straw can I remove straw and chug instead?
