Q: Does this tripod come with everything seen in the first picture. the adjustable bowl head that allows some angle if on uneven ground?

The RT90C i purchased in December 2019 did have everything included in the first picture. However, when i unboxed the tripod, I initially missed finding the bowl adapter head. It was stored in a pouch in bottom of the carrying case.

Q: I’m curious to know if the adjustment collars/knobs used to adjust the length of each leg on this tripod are rubber coated or knurled aluminum?

I've only seen one of these in person (mine that I purchased) and it's knurled aluminum. Feels very solid, well made, locks the legs well. I've not given it any hard use in competition yet, but here in Colorado it's already seen Sun, Sand, and Snow and has been fantastic to use. I plan on getting a 2nd one for my Nikon camera setup.

Q: Is the diameter of the hole and top plate the same as the artcise as90c?

No, this is an intermediate size tripod, and the top plate and bowl adapter are smaller than the as90c.
