Is the second hand movement smooth or jerky?

Does clock tick out loud?

No. Although it does have a sweep second hand it is very quiet

is there a way to set the time?

Yes there is. Under the #3 on the side of the clock is a button much like on a wrist watch. Pry the clock out of it's fixture and pull the button out to set the time. The button is slightly recessed so the clock can fit in a round opening. Let me add that we are pleased with our mini clock. Look great and keeps time.
