why do you need extra nibs? is it really something that wears out?

Yes they do wear down with constant use... but they last a long time.

Would these nib replacements be compatible with the intuos tablet cth-680? i can't find any nibs that are explicitly compatible with it.

The nibs are in the pen base. That is the cylindrical base the pen sets in. unscrew the bottom of the base and you will find the extra pen nibs.
