why do all the photos show underwear below model's belly button but in reality they all go almost to the bra line?

The under wear I purchase as you mentioned is not true. Choosing the size , style and materials is the key on buying underwear. It is costly but worthy. I hope this help.

If I’m a size 7/large in panties with a waist of 43 and hips of 49, what would be the best size for me?

My wife is 39” and 41” respectively and I ordered compared to the size chart. You have a similar size ❤️ ... she says they are her favorite panties now. Originally she thought they would go up too high on her belly, but they look sexy on her and feel very comfortable. Good luck!

I have purchased another brand, size 3xl that fit great. What size should I get with this brand?

This brand sizes are smaller than the same size Pokarla brand. I bought this brand thinking it would fit perfectly as same size.. it was very snug, not as comfortable. You would have to order a size up.

Which is right. It says 100% cotton under product details, is this accurate or are they 5% spandex?

They are 95% cotton, 5% spandex. There is a tag located inside of them which states that.
