How tight does the strap have to be for it to work?

I keep mine fairly tight, but my nurse recommended switching out to different masks during the day, so as not to leave demarcation lines. This one is comfortable and very adjustable, so just Velcro it as tight or as loose as you want it.

Yes please, it is It has no harm at all

It looks small, how much does it measure from end to end when you lay it flat?

It is 24 inches end to end but it has a lot of stretch.
