Q: How is using this tester preferable to hooking up your multimeter in series with alligator clips and a blade fuse soldered inline with your leads?

It helps because it has inline fuses which will protect your voltage meter. When using your meter directly you risk damaging your meter.

Q: what gauge are the cables that connect to the battery and terminal. Can I start and run the vehicle with the tester connected?

No, they are a light gauge wire. Too light of wire to handle load of starter.

Q: Does it come with instructions?

not really, you have to look at a video from mac or matco which has same tool just their name. its hard the first time ,but you can figure it out after you start to use it.

Q: This seems useless. Why not use a clamp on dc meter?

I liked it because it has a fuseable link on both leads and you can connect your probe to it. I blew a couple fuses while trying to track down a short and it prevented an damage to my rather inexpensive multimeter. It's designed so that clamp it to the battery directly. If you already have leads with clamps then yes this would probably not be helpful to you. It worked great for me fixing a slow drain on our Jeep. I mostly found it helpful while pulling fuses until I found the circuit that was causing the trouble. I could have spent a fortune having it in the shop so I figured spending a couple bucks on this was a much better option.
