Q: Does it comes with the power supply?

It does not come with power supply. This unit does not need external power if it is connected with USB cable, the power is supplied through USB cable. It only needs external power when parallel port cable, or PS/2 prot cable is used.

Q: What is the mfr part# for the scanner? what is the mfr part# for the base? does the scanner come with the scanner battery pack?

The kit includes the scanner (with a battery packed inside of it), a cradle/base for charge and cummunication, and a usb cable.

Q: Does this scanner capture QR codes?

I tried mine and it would not scan QR codes

Q: When connected to my usb ports(i tried all of them) the cradle led does not stay illuminated and the unit doesn't charge or work. any suggestions?

I don't have any problems with mine. It is just plugged into the usb port on my computer and it charges and the light is on. Perhaps you're base unit s defective.
