Does this tool work on regular paper?

It does not work with regular paper at all. Or watercolor paper, which I also tried. This instrument is specific to leather, which is does very well.

Would this leave a clean hole in cardboard?

Yes it should, it leaves a clean hole in a leather belt so there should be no reason for it not to leave a clean hole in cardboard
