With the ratcheting feature I can make the fit skin tight around the front and back of my scalp enough to induce pain, and extreme tightness. As a tool to block spit or phlegm from a patient it is excellent. However I find myself having to shout at a high volume in order to make most patients hear me. I'm a welder and I use this thing every day... sometimes for hours a day. I added a pair of Walker low-profile shooting ear muffs and just zip tied them to the head band, so now it's also hearing protection.
