Is this shipped in a plain box - one that does not tell what is in the box

No, they ship in a box that shows the product. They used to ship in a plain brown box but changed that and so it was embarrassing to say the least.

I don't know if they have a sizing chart. The regular length is roughly 9 in long and the long length is 12 in. The advantage of a long is that it covers more and accidents aren't at the end of the pad.

What is the difference between ultimate and maximum?

The maximum are #5 and the ultimate are #6. #6 are longer.
