Does it take special 2" adapter for deep well single pipe application? If so, please let me know

I used it as a two pipe deep well application with the injector at the bottom of well, for single pipe, I believe you install the injector ( not included ) at the pump, this allows you to run just one pipe down the well.

Is it capable at a depth of 100 feet ?

According to the manufacturer's website, this pump with a DW40E-1520 injector (sold seperately) would work. Spec sheet is 4.3 GPM @ 100 feet or 3.7 GPM @ 110 feet. The DW Minimum Operation PSI is 39-45. Hope this helps! Jack's Country Store

what flow rate could be expected with a water depth at 25 ft with 50 psi discharge pressure?

The Gallons Per Minute flow rate will vary depending on the ejector attached to the pump, if you would like detailed information on available ejectors and their output could I kindly suggest calling our USA based installation support line @ 1-800-742-5044, they will help you with choosing the right ejector for your needs. I can also suggest looking at our JHU10S which is the optimal version of this pump with a shallow-well ejector and can provide you with 9.0 GPM @ 25 feet with 50 PSI.

Besides the ejector, does the pressure gauge need to be purchased separately?

Yes: A common item available at any hardware store. 0 to 100 psi is what I purchased. Cost: 5 to $ 15.00
