It doesn't have a lot of pouches, but that does help it fit under your pant leg without a problem. I don't wear any special wide cut pant leg, just strait cut, and it fits fine when loaded with a CAT tourniquet, gloves, flat-folded tape, QuikClot gauze, and an added SWAT-T tourniquet/dressing. I have ran laps and ridden a bicycle with this on with no problems. One downside: The MOLLE attachment part works fine for weaving MOLLE, but I've found any MOLLE attachment becomes too bulky to fit under the pant leg anymore. I used it instead to mount elastic shock cord to create a holder for a SWAT-T, as you can see by the pictures. Ultimately, while this is not a large enough kit to fit a full IFAK around your ankle, it has enough space to fit necessities comfortably and has so far stood up to over 6 months daily use for me.
