FYI I'm 6ft 205 lbs male and the small/medium fits great as you'll want it to be tight. Velcro holds nicely. So just watch out with leaning and bending over. Its technically called acute lumbar back spasm and its excruciating. I'm still healing from the lower back now at week 5. So watch out and use that grabber tool to grab things down low. Raise your foot up to a bench or chair to put shoes on etc. Docs didn't warn me about this but I am. If you only need basic support than the belt is very good without the insert. Where it really shines is post-op support. Here the belt without the insert provides excellent support and comfort particularly if post-op swelling. The Velcro is top notch and very durable. In the post-op time, it is very adjustable. I give it only 4 stars as the insert is a problem, but 5 stars (if I could at the same time) for post-op use.
