Has anyone experienced this locking on its own, and being locked out?

I would be difficult for this device to lock on its own since you need to lift it out of the locked position to open and close it.

Does it come with hardware?

Yes it comes with mounting screws.

Do the door & door frame need to be even for this to work? My door frame is at least .5" higher than my door.

The door jamb needs to be next tot he door where it is installed.

Can this be installed on a steel door frame?

Yes, but, you might need to supply the correct screws for the steel door. the screws included are for wood.
