If I put just one regular size business card in slot, would it be loose or anything like it would fall out?

No it will not fall out. Excellent organizer.

Can additional refill pages and dividers be purchased?

I only received it last week, so I don't know.

Does it come with the card holder pages?

I love this little holder! The pages are fragile but, I just love it anyway! I used my label maker to label the indexes! I use it for work!
